Instructions

1. Access our Patient Portal
2. Sign in with User ID provided and password 1234
3. Click "I Agree" to "Terms of Use"
4. Click "I Agree" to "Notice of Privacy"
5. Either update demographic page, or select "no change"
6. Click on "History" from column on left
7. Save and Close

New patients

• Please download, complete and fax/mail the Medical Information Release form to your current physician(s).
